Investigation report on serious incident of dual hydraulic failure of A330-243 passenger aircraft published

At 12.23pm, the aircraft took off from the South Runway of HKIA to Denpasar, Indonesia. Upon reaching 3 460 feet above mean sea level on the climb, the electronic centralised aircraft monitor issued warnings and associated messages indicating relatively low pressure in the Green and Blue hydraulic systems.
The flight crew carried out the corresponding emergency procedures and subsequently declared an emergency, deciding to return to HKIA for a full emergency landing. The aircraft landed safely on the South Runway and stopped abeam taxiway J3 with the No. 5 tyre burst. No one was injured.
The investigation was conducted by a team of professional investigators in adherence to international standards established by the International Civil Aviation Organization. The investigation concluded that this serious incident was caused by the complete detachment of one of the manual valves in the ground service manifold of the Green hydraulic system due to the fatigue failure of the four attachment screws of the valve, and the subsequent puncture in the return pipe of the Blue hydraulic system made by the detached valve.
"The investigation report was published by the AAIA with the objective of identifying the circumstances and causes of the serious incident with a view to preventing recurrence," an AAIA spokesperson said.
