DH reports missing worksheets containing clients' personal data

       The Department of Health (DH) announced today (September 5) a case of lost personal data.

       According to a preliminary enquiry by the DH, an envelope containing a set of worksheets bearing information of 191 members of Tung Chung Elderly Health Centre was found to be missing on August 30, 2016. The data contained in the worksheets included names, Hong Kong Identity Card numbers, membership numbers, and other coded health assessment data of elderly members who received health assessment at the centre in 2015.

      Investigation is ongoing to locate the envelope and to find out the exact cause of the incident. 

      The DH is contacting the clients involved by phone and mail to alert them to the incident.

      The DH has reported the incident to the Office of the Privacy Commissioner for Personal Data and the Police.

     "We extended apologies to those clients affected for the inconvenience caused. The DH is very concerned about the incident and will conduct a thorough investigation with a view to avoiding recurrence of similar incidents. We will also step up efforts in reminding staff members of the importance of protection of personal data and privacy," a DH spokesman said.
