Kwong Wah Hospital announces sentinel event

The spokesperson for Kwong Wah Hospital (KWH) made the following announcement today (April 29) regarding a sentinel event:
A pregnant woman had an antenatal check-up at KWH on April 14 and was found of having hypertension and symptom of pre-eclampsia. She was admitted for observation and treatment of hypertension. The woman was stable during the admission and was granted home leave every night and reminded to return to ward on the next day.
The woman left the ward on April 24 and returned to ward on April 27 but hypertension continued and deteriorated. She was then transferred to the Labour Ward for close monitoring. In view of the deteriorating condition, the doctor recommended early delivery by caesarean section. The woman developed fever and progressive shortness of breath at night time of the same day. She further deteriorated around the midnight of April 28 and developed acute pulmonary oedema. The husband of the patient was informed. Emergency caesarean section was performed with baby delivered.
The woman had cardiac arrest afterwards. Resuscitation was performed successfully and she was transferred to the Intensive Care Unit with ventilation support and medication. The woman suffered from several cardiac arrests again and finally succumbed in the afternoon at around 3pm after resuscitations were in vain. Doctor had met with her husband to explain about the process of delivery and treatment.
The hospital expresses its deepest condolences to the family and would continue to closely communicate with the family to provide all necessary assistance. The case had been referred to the Coroner for further follow-up.
The hospital has reported the incident to Hospital Authority Head Office via the Advance Incident Reporting System, and would identify the root causes of this incident.
